Wisdom Tooth Removal: When Is Wisdom Tooth Extraction Necessary?

Wisdom teeth — the third molars at the very back of the mouth — are the last teeth to develop. Most people have four, typically emerging between ages seventeen and twenty-five. Many jaws lack enough room for them, which causes the teeth to become impacted. Not all wisdom teeth need removal, but many do.

Seven Reasons Removal May Be Necessary

1. Gum disease. Impacted wisdom teeth create pockets where bacteria accumulate, leading to periodontal disease around neighboring molars.

2. Tooth decay. The far back of the mouth is hard to clean, and deep decay that cannot be restored is grounds for extraction.

3. Tooth crowding. Even partially emerged wisdom teeth can push adjacent teeth and disrupt alignment — a particular concern after orthodontic treatment.

4. Damage to adjacent teeth. A wisdom tooth growing at an angle can press against the second molar and cause decay or structural damage.

5. Jaw damage. Cysts can form around impacted wisdom teeth and, if left untreated, can hollow out the jaw and damage nerves.

6. Sinus issues. Upper wisdom teeth can grow toward the sinuses, contributing to pain, pressure, and congestion.

7. Pericoronitis. Infection of the gum flap covering a partially erupted wisdom tooth can cause swelling, pain, a bad taste, and difficulty opening the mouth.

When Removal Isn't Needed

If a wisdom tooth is fully erupted, properly aligned, healthy, and easy to clean, routine monitoring at checkups is usually all that is required.

The Extraction Process

Treatment begins with a consultation and either a panoramic X-ray or a 3D cone beam CT scan. Local anesthesia or IV sedation is used, and the extraction is either simple or surgical depending on the level of impaction. Most patients experience the strongest discomfort in the first three days, follow a soft-food diet for about a week, and see the socket heal over three to four weeks.
